I think it’s some other object, mainly because I wouldn’t put it past Sir to do whatever possible to drive Boy up a wall that way.
And I just have to say . . . I just love how they’ve brought boy to the point of drooling with all this play. I’d be surprised if Boy can see anything clearly at all.
While within the context of a story, it’s possible, I can say for certain that objects, even those made specifically for the purpose, do not feel like real cocks. They may both feel great, but they feel distinctly different.
I think it’s Sir, because of the hands in the second panel. They look as if they have a lot of hair and the Butler’s have none.
d) Both Sir & Butler
Because if Boy is so far gone as he says he is, it means he will be incredibly relaxed and might be able to accommodate two cocks without realizing it. There is a lot of black in first panel which I think is Butler pouring lube and is crouched on top, whereas Sir is behind Butler.
(I may be biased because DP is hnnnngh! And I think Boy getting penetrated by both Butler and Sir might throw him off the edge into utter bliss?)

Tab has previously mentioned the decision to not show genitals as allowing the reader to imagine whichever parts they choose to see there. It adds a new dimension for me to ponder that Sir or Butler’s “cock” they are asking about could be imagined as a particular strap-on (though that may not be historically accurate, I’m still intrigued).
Such things definitely existed. Possibilities!! Although I personally feel that the boy is the trans baby here, but you could definitely be right.
